Guidelines for filling up the SOTAX Scholarship Application form
Ensure that you have scanned copies of all the following documents ready before you start filling up the application form.
- Report Card of the academic year 2024-2025 (.jpg or .pdf file of less than 1MB size)
Please note: The academic year, name of the school, board, passed standard, child’s name, parent’s name and total marks must be clearly visible on the report card.
Name the file as reportcard.jpg or reportcard.pdf
- The coloured or painted picture of SOTAX DISSOLUTION TESTER (.jpg file of less than 1MB size.
Name the file as colorsketch.jpg
Download and take print of the SOTAX DISSOLUTION TESTER and colour it. Any type of colours can be used.
- Copies of all the certificates or testimonials of Extra Curricular activities of academic year 2024-2025. (Each certificate as .jpg file of less than 1MB size.
Name the files as shown below
- SchoolParticipation_01.jpg, SchoolParticipation_02.jpg and so on
- SchoolMerit_01.jpg, SchoolMerit_02.jpg and so on
- DistrictParticipation_01.jpg and so on
- DistrictMerit_01.jpg and so on
- StateParticipation_01.jpg and so on
- StateMerit_01.jpg and so on
- NationalParticipation_01.jpg and so on
- NationalMerit_01.jpg and so on
Please note that the application may get Rejected or disqualified if,
- The course completion certificates, online quizzes, certificates not properly attested will not be considered for evaluation.
- Application filled with wrong information
- Paintings of the siblings if found to be identical
Once you have all the documents ready as above and, you are ready with all the correct details of the student and parent, start filling up the application form:
Student details:
- Enter Student’s First name, Middle name and Last name
- Enter full name as you require it on the Certificate
- Enter date of birth of the student
- Select student’s gender
- Enter Residence Address, City, State and Pin code
- Select the passed-out Class/Standard of the last academic year: 2024-2025
- Aggregate Percentage of marks achieved. Whole number or with 2 decimal places
- Upload Last Report card in .jpg or .pdf file format.
- Enter Name of the School, City, State
Parent details:
- Enter Parent’s First name & Last name
- Select the relation of the person to the student
- Enter name of the pharmaceutical company where parent is working
- Enter His/her designation and department
- Office address, City, State and Pin code
- Office email id, Personal email id(must: for communications), Mobile number and Residence/alternate Phone
Certificates:
- Enter the number of Participation & Merit certificates you have under each level, School, District, State & National
- Upload all the certificate files, one by one in .jpg format (certificates of the period April 2024 to March 2025 only).
- Upload the Color sketch file in .jpg format
Click on Submit button
If your form is not getting submitted, scroll up and check if there are any errors (marked red) or blank fields where data is required. Ensure all the information is correct before you click on Submit.
On successful submission of the SOTAX Scholarship Application form, you will receive a Thank you message, simultaneously an autogenerated acknowledgement will be sent on the personal email (as provided by you while filling the application form).
